The Saudi authorities have been criticised after a crush of two large columns of people during the Hajj pilgrimage - the latest in a line of disasters around Mecca.
Millions of pounds have been spent on expanding the shrine area in Mecca, but, as ITV NewsInternational Affairs Editor Rageh Omaar reports, the precautions put in place were not able to prevent the latest in a line of catastrophes.
